OBSTRÜKTİF UYKU APNESİNİN EŞLİK ETTİĞİ VENTRİKÜLER EKSTRASİSTOL VAKASINDA AKUPUNKTUR TEDAVİSİNİN ETKİSİ
Abstract
ÖZET
Giriş ve Amaç: Ventriküler Ekstrasistol (VES) toplumda yaygın olarak görülen ve genelde asemptomatik seyreden ekstra kalp vurularıdır. Asemptomatik vakalarda herhangi bir tedaviye gerek duyulmazken, ciddi ritim bozukluklarında ya da çarpıntı, sersemlik, göğüs ağrısı, dispne, presenkop gibi semptomlara neden olanlarda antiaritmik ilaçlar ya da kateter ablasyonu temel tedavi olarak kabul edilmektedir. Bu tedavilerin etkisiz kaldığı ya da uygulanamadığı hastalar alternatif tedavi arayışlarına yönelmektedir.
Bu olgu sunumunun amacı medikal tedaviye cevap vermeyen ve Obstrüktif Uyku Apnesi Sendromu (OSAS) nedeniyle kronik nokturnal hipoksiye maruz kalan VES tanılı bir hastada akupunktur tedavisinin başarılı sonuçlarını sunmaktır.
Gereç ve Yöntemler: Hastanın Geleneksel Çin Tıbbı’na (TCM) göre nabız ve dil muayeneleri yapıldı. Buna göre haftada bir olmak üzere toplam 11 seans Nöropsikopatojen Eliminasyonu-Basic (NPPE-Basic) bozucu alan tedavisi, klasik TCM ve Master Tung akupunktur tedavileri uygulandı.
Bulgular: Tedavi sonrasında hastanın halsizlik, yorgunluk, çarpıntı, gündüz uyku hali, öfke atakları gibi OSAS ve VES kaynaklı klinik semptomları düzeldi. HOMA-IR değeri 5.3’den 2.44’e düşerek insülin direnci kayboldu. Elektrokardiogram’da (EKG) VES saptanmadı.
Sonuç: Akupunktur tedavisi diğer tedavilere yanıt vermeyen VES ve OSAS vakalarında alternatif bir seçenek olabilir.
