Antiphosholipid Antibodies

Abstract

In recent years, antiphospholipid antibodies have received much attention. There are two well described groups of antiphospholipid antibodies, anticardiolipin antibodies, and lupus anticoagulant.

The strong correlation between presence of the lupus anticoagulant and anticardiolipin antibody, suggests that these are cross-reacting antiphospholipid antibodies.

Elevated anticardiolipin antibody levels have been detected in autolmmune diseases such as systemic lupus erythematosus and rheumatoid
arhritis. They have also been noted in pregnancy and occacionally in patients without autoilmmune disorders.

Most of studies have shown that the associations of the anticardiolipin antibodiy and the closely related lupus anticoagulant with thrombosis, recurrent fetal loss and thrombocytopenia. It is uncertain whether antiphospholipid antibodies play a pathogenetic role in thrombosis or are «markers» for thrombotic disease, Immunosupressive therapy has been discussed in patients with elevated anticardiolipin antibody levels,

Abstract

Antifosfolipit antikor ailesinden başlıca iki grubu iyi belirlenmiştir. Bunlar lupus antikoagülanı ve antikardiyolipin antikorudur. Antikordiyolipin
antikorunun varlığını, biyolojik false pozitif standart sıfiliz testi yansıtabilirse de, daha hassas bir yöntem olan standardize edilmiş ELİSA ile tayin etmek mümkündür.

Sistemik lupus eritematozus ve romatoid artrit gibi otoimmün hastalıklarda, yüksek antikardiyolipin antikor seviyeleri saptanmıştır. Otoimmün olmayan hastalıklarda da antikordiyolipin antikor pozitifliği gösterilmiştir. Çalışmaların çoğunda, yüksek antikardiyolipin antikor ve onunla yakın ilişkili olan lupus antikoagülanın trombozis. rekürrent fetus kaybı ve trombositopeni ile beraberlik gösterdiği dikkati çekmiştir. Fosfolipitlere karşı oluşan bu antikorların trombozisde patogenetik bir rolü olup olmadığı konusu kesin bilinmemektedir. Yüksek antikardiyolipin antikorları saptanan hastalarda immunosüpresif
tedavinin önemi ve yeri tartışmalıdır.
