Unmet Healthcare Needs and Socioeconomic Determinants Among Individuals Aged 65 and Over in Türkiye: An Assessment for the Period 2019–2022
Abstract
Aim: This study aims to evaluate the challenges faced by elderly individuals in accessing healthcare services due to the progressively aging population in Turkey. It examines the unmet healthcare needs of individuals aged 65 and over in Turkey, their association with socioeconomic factors, and changes over time.
Materials and Methods: The research utilizes data from the Turkish Statistical Institute’s Health Surveys for the years 2019 and 2022. Data on unmet healthcare needs of individuals aged 65 and over were analyzed using Chi-square tests and binary logistic regression.
Results: In relation to delays in appointment scheduling, income (OR = 1.179, p = 0.003), disability status (OR = 1.230, p = 0.005), and general health status (OR = 0.763, p < 0.001) were found to be significant determinants. Regarding access to healthcare services, lack of social support (OR = 0.566, p = 0.001), income level (OR = 0.743, p < 0.001), and geographical region (OR = 1.066, p < 0.001) emerged as important factors. In terms of access to medical care due to financial difficulties, income level (OR = 0.573, p < 0.001), general health status (OR = 0.613, p < 0.001), and disability status (OR = 2.595, p < 0.001) were prominent.
Conclusion: The findings indicate that structural and socioeconomic factors are key determinants in the elderly population’s access to and utilization of healthcare services. In order to ensure equity in healthcare, it is essential to develop policies that address the specific needs of elderly individuals.

Türkiye’de 65 Yaş ve Üstü Bireylerde Karşılanmayan Sağlık Hizmeti Gereksinimleri ve Sosyoekonomik Belirleyiciler: 2019-2022 Dönemine Yönelik Bir Değerlendirme
Abstract
Amaç: Türkiye nüfusunun giderek yaşlanması nedeniyle sağlık hizmetlerine erişim ile ilgili olarak yaşlı bireylerin karşılaştıkları zorlukların değerlendirilmesi bu çalışmada amaçlanmaktadır. Türkiye'de 65 yaş üzeri bireylerin karşılanmamış sağlık hizmeti gereksinimi ve bunun sosyoekonomik etmenlerle ilişkisi ve zaman içindeki değişimi incelenmektedir.
Gereç ve Yöntem: Araştırmada, Türkiye İstatistik Kurumu’nun 2019 ve 2022 yıllarına ait Sağlık Araştırması veri setleri kullanılmıştır. 65 yaş ve üzeri bireylerin karşılanmamış sağlık ihtiyaçlarına ilişkin veriler Ki-kare testi ve binary lojistik regresyon yöntemi ile değerlendirilmiştir.
Bulgular: Randevu sürelerinde yaşanan gecikmede gelir (OR = 1,179, p = 0,003), engellilik durumu (OR = 1,230, p = 0,005) ve genel sağlık durumu (OR = 0,763, p<0,001) anlamlı belirleyiciler olarak bulunmuştur. Sağlık hizmetlerine ulaşımda ise sosyal destek eksikliği (OR = 0,566, p = 0,001), gelir düzeyi (OR = 0,743, p<0,001) ve coğrafi bölge (OR = 1,066, p <0,001) önemli belirleyicilerdir. Ödeme güçlüğüne bağlı tıbbi bakıma erişiminde gelir düzeyi (OR = 0,573, p<0,001), genel sağlık durumu (OR = 0,613, p<0,001) ve engellilik durumu (OR = 2,595, p<0,001) öne çıkmıştır.
Sonuç: Elde edilen bulgular, yaşlı bireylerin sağlık hizmetlerine erişiminde ve hizmetleri kullanabilmelerinde yapısal ve sosyoekonomik faktörlerin belirleyici olduğunu göstermektedir. Sağlık hizmetlerinde eşitliği sağlamak için yaşlı bireylerin ihtiyaçlarına yönelik politikalar geliştirilmesi gereklidir.
