Organ Nakli Hastalarında Stresle Başa Çıkma ve Algılanan Sosyal Desteğin Travma Sonrası Büyüme Üzerine Etkisi

Abstract

Giriş: Organ nakli, psikolojik travmaya neden olabilen stresli bir deneyimdir. Bu nedenle organ nakli alıcıları işlemden sonra travma sonrası stres bozukluğu görülmesi açısından risk altında olabilirler. Bununla birlikte, alıcıların Travma Sonrası Büyüme (TSB) olarak bilinen olumlu değişiklikler yaşaması da mümkündür. Bu olumlu değişikliklerin varlığı çeşitli faktörlerden etkilenebilir.
Amaç: Çalışma, organ nakli alıcılarında TSB düzeyini ve ilişkili faktörleri değerlendirmek ve stresle başa çıkma ile algılanan sosyal desteğin TSB üzerindeki etkisini incelemek amacıyla yürütüldü.
Yöntem: Bu tanımlayıcı çalışma Mart 2022 ile Haziran 2022 tarihleri arasında gerçekleştirilmiş olup, bir devlet üniversitesinin Sağlık Araştırma ve Uygulama Merkezi ile özel bir hastanenin organ nakli merkezlerinde takip edilen 133 hastayı kapsamaktadır. Veriler Demografik ve Klinik Özellikler Formu, Travma Sonrası Büyüme Envanteri (TSBE), Stresle Başa Çıkma Ölçeği (SBÇÖ) ve Çok Boyutlu Algılanan Sosyal Destek Ölçeği (ÇBASDÖ) kullanılarak toplandı. Verilerin değerlendirilmesinde yüzdesel dağılım, ortalama, bağımsız t-testi, tek yönlü varyans analizi, Pearson korelasyon testleri ve adımsal regresyon analizi olarak kullanıldı.
Bulgular: Ortalama TSBE puanı 75.65±20.10 (yüksek düzey) idi. Stresle başa çıkma (r = 0.35, p<.01) ve algılanan sosyal destek (r = 0.29, p<.01) ile TSB arasında pozitif yönde ve ileri derecede anlamlı korelasyon bulundu. Adımsal regresyon analizi TSB'yi etkileyen faktörleri şu sırayla gösterdi: Problem odaklı başa çıkma (%9), sosyal destek arama (%6), aileden algılanan sosyal destek (%6), arkadaşlardan algılanan sosyal destek (%3) ve sorunlardan kaçınma (%2).
Sonuç: Sonuç olarak, bulgular stresle başa çıkma ve algılanan sosyal desteğin organ nakli alıcılarında TSB gelişimine katkıda bulunan faktörler olduğunu göstermektedir.

The Effect of Coping with Stress and Perceived Social Support on Post-Traumatic Growth in Organ Transplant Patients

Abstract

Background: Organ transplantation is a stressful experience that can cause psychological trauma. Transplant recipients may therefore be at risk of developing post-traumatic stress disorder after the procedure. However, it is also possible for recipients to experience positive changes, known as Posttraumatic Growth (PTG). The presence of these positive changes may be influenced by various factors.
Objectives: The study was conducted to assess the level of PTG and associated factors in organ transplant recipients and to examine the effect of stress coping and perceived social support on PTG
Method: This descriptive and cross-sectional study was conducted between March 2022 and June 2022 and included 133 patients followed at the Health Research and Application Center of a state university and the organ transplantation centers of a private hospital. Data were collected using the Demographic and Clinical Characteristics Form, Posttraumatic Growth Inventory (PGI), Stress Coping Scale (SCS) and Multidimensional Perceived Social Support Scale (MSPSS). Data were analyzed using percentage distribution, mean, independent sample t-test, one-way analysis of variance, Pearson correlation tests and stepwise regression analysis.
Results: The mean PTGI score was 75.65±20.10 (high level). Positive and highly significant correlations were found between coping with stress (r = 0.35, p<.01) and perceived social support (r = 0.29, p<.01) and PTG. Stepwise regression analysis showed the factors affecting PTG in the following order: Problem-focused coping (9%), seeking social support (6%), perceived social support from family (6%), perceived social support from friends (3%), and avoiding problems (2%).
Conclusion: In conclusion, the findings suggest that stress coping and perceived social support are factors contributing to the development of PTG in organ transplant recipients.
