DETERMINANTS OF HEALTHCARE EXPENDITURES: A QUANTILE REGRESSION APPLICATION

Abstract

This study was carried out to determine the factors affecting health expenditures and to propose solutions for these factors. Although there are many factors affecting health expenditures, the variables of income, education expenditures, alcohol and tobacco use rates, proportion of population over 65, proportion of urban population, proportion of female population and number of physicians of 138 countries in 2019 from the World Bank database are considered within the scope of the study. In the study, first classical least squares regression and then quantile regression analyses were used. The results obtained are interpreted at 95% confidence level. As a result of the analyses, it was found that in countries in quantiles 0.25, quantile 0.50 and quantile 0.75, per capita income, the share of education expenditure in gross domestic product and the proportion of population over 65 years of age increased, and per capita health expenditures increased in a statistically significant manner. No statistically significant relationship was found between alcohol and tobacco use rates, urban population ratio, female population ratio and number of physicians and health expenditures.

Abstract

Bu çalışma sağlık harcamalarını etkileyen faktörlerin belirlenmesi ve belirlenen bu faktörlere yönelik çözüm önerileri getirilmesi amacıyla gerçekleştirilmiştir. Sağlık harcamalarını etkileyen birçok faktör bulunmakla beraber çalışma kapsamında Dünya Bankası veri tabanından 2019 yılına ait 138 ülkenin gelir, eğitim harcamaları, alkol ve tütün kullanım oranları, 65 yaş üstü nüfus oranı, kentsel nüfus oranı, kadın nüfus oranı ve hekim sayısı değişkenleri ele alınmıştır. Çalışmada, öncelikle klasik en küçük kareler regresyonu ve ardından kantil regresyon analizleri kullanılmıştır. Elde edilen sonuçlar %95 güven düzeyi üzerinden yorumlanmıştır. Yapılan analizler sonucunda kantil 0,25, kantil 0,50 ve kantil 0,75’te yer alan ülkelerde kişi başına düşen gelirin, eğitim harcamasının gayrisafi yurt içi hasıla içindeki payının ve 65 yaş üstü nüfus oranının artmasıyla kişi başına düşen sağlık harcamalarının da istatistiksel olarak anlamlı bir şekilde artış gösterdiği tespit edilmiştir. Alkol ve tütün kullanım oranları, kentsel nüfus oranı, kadın nüfus oranı ve hekim sayısı ile sağlık harcamaları arasında istatistiksel olarak anlamlı herhangi bir ilişkiye ise rastlanmamıştır.
