Yineleyici Majör Depresif Bozuklukta Yürütücü İşlevler ve Çalışma Belleğinin Rolü

Abstract

Majör Depresif Bozuklukta (MDB) tedaviye uyumsuzluk ve tekrarlayan ataklar sık karşılaşılan bir durumdur. Bu çalışmada MDB’de atak sayısı, tedavi uyumu, çalışma belleği, inhibisyon, perseverasyon, algılanan stres düzeyi ve karar verme arasındaki ilişkinin araştırılması amaçlanmıştır. Araştırmada Tedavi Uyum Ölçeği, Harf-Sayı Dizisi Alt Testi, Stroop Testi TBAG Formu, Wisconsin Kart Eşleme Testi, Algılanan Stres Ölçeği-14 ve Iowa Kumar Testi kullanılmıştır. Analiz için Düzenleyici Analiz ve Yapısal Eşitlik Modellemesi kullanılmıştır. Araştırmada MDB’de atak sayısının tedavi uyumu üzerindeki etkisinde çalışma belleğinin düzenleyici rolü olduğu bulunmuştur. Aynı zamanda çalışma belleği inhibisyon, perseverasyon, algılanan stres düzeyi ve karar verme işlevlerini yordamaktadır. Devamında çalışma belleği, atak sayısının inhibisyon ve algılanan stres düzeyi üzerindeki etkisinde aracı rol üstlenmektedir. Son olarak MDB hastalarında çalışma belleğinin perseverasyon ve karar verme üzerindeki etkisinde, tedavi uyumunun aracı rolü olduğu görülmüştür. Elde edilen veriler sonucunda MDB tedavisinde hastaların ilaç tedavisine uyumlarını sağlamanın yanı sıra çalışma belleği performanslarını geliştirmeye yönelik bilişsel müdahale programlarının geliştirilmesinin klinik bir gereklilik olduğu düşünülmüştür.
