Öz
Baypass, akut üst ekstremite iskemisinde nadiren uygulanan revaskülarizasyon tekniğidir. Ekstremite kaybını ve morbiditeyi önlemek için zamanında tanı ve müdahale yapılmalıdır. Bu olguda brakiyal embolektomi sonrasında ekstremite iskemisinin devam etmesi nedeniyle yapılan brakiyo-brakiyal baypas cerrahisini sunmayı amaçladık. Olgu, akut başlangıçlı nefes darlığı ve sol kol ağrısı ile acil servise başvuran 74 yaşında erkek hastaydı. Brakiyal arterde oklüzyon saptandı. Brakiyal embolektomiye rağmen yeterli akım sağlanamadı. Brakiyo-brakiyal baypas yapıldı ve hızlı klinik iyileşme sağlandı.

Öz
Bypass is a rarely applied revascularization technique in acute upper extremity ischemia. Timely diagnosis and intervention must be made to prevent limb loss and morbidity. In this case, we aimed to present brachio-brachial bypass surgery performed due to persistence of limb ischemia after brachial embolectomy. The case was a 74-year-old male patient who presented to the emergency department with acute onset of shortness of breath and left arm pain. Occlusion was detected in the brachial artery. Despite brachial embolectomy, adequate flow could not be achieved. Brachio-brachial bypass was performed and rapid clinical recovery was achieved.
