Tibia Cisim Kırığına Eșlik Eden Ayak Bileği Kırıkları Klinik Sonuçlarımız ve Literatürün Gözden Geçirilmesi

Öz

Amaç: Tibia cisim kırıkları sıklıkla karșılașılan uzun kemik kırıklarındandır ve beraberinde tibianın proksimal ve distal eklemlerini içeren kırıklar görülebilir. Tibia cisim kırığına eșlik eden ayak bileği kırıkları değerlendirildiğinde posterior malleol kırıklarının cisim kırığına en sık eșlik eden kırık tipi olduğu bilinmektedir. Bu çalıșmamızda tibiacisim kırıklarına eșlik eden ayak bileği kırıklarının çeșitleri, sıklığı ve bunların tibia cisim kırığının yerleșimi, etyolojisi ve kırığın șekli ile olan ilișkilerini göstermek amaçlandı.

Gereç ve Yöntem: Hastanemiz acil servisine Ocak 2015 - Haziran 2017 tarihleri arasında bașvuran tibia cisim kırığıbulunan hastaların verileri retrospektif olarak tarandı ve toplamda 79 tibia cisim kırığı olan hastaya ulașıldı. 43 hastada ayak bileği kırığının tibia cisim kırığına eșlik ettiği görüldü. Bunlardan 9 hastada distal fibula, 14 hastada posterior malleol, 21 hastada lateral malleol ve 4 hastada da medial malleol kırığı görüldü.

Bulgular: İstatistiksel analiz sonrasında kırık mekanizması ile ayak bileği kırığının tipi arasında anlamlı ilișki olmadığıgörülmüștür. Benzer șekilde kırık seviyesinin ayak bileğikırığı için bir etken olmadığı görülmüș ancak spiral cisim kırığına, ayak bileği kırıklarının daha sık eșlik ettiği görülmüștür.

Sonuç: Tibia cisim kırığının etyolojisi ve yerleșimlerinden bağımsız olarak ayak bileğinde eș zamanlı kırık olabileceği, özellikle spiral șekilli tibia kırığı varlığında bu birlikteliğin daha yüksek olasılıkta olduğu görüldü.

Ankle Fractures Associated With Tibia Shaft Fractures Our Clinical Results and Review of Literature

Öz

Aim: Tibia shaft fracture is a common long bone fracture which might be seen along with tibia plateau, malleol and fibula fractures. It is known that posterior malleol fractures are the most frequent fracture type that accompany tibia shaft fractures. In this article, we aim to show the types, frequency, etiology and the fracture configurationof ankle fracture that accompany tibia shaft fractures.

Materials and Methods: The patients who were admitted to the Emergency Department between January 2015 and June 2017 were included in the study. All the data was extracted retrospectively. A total of 79 patients who had tibia shaft fracture were examined. 43 patients had ankle fracture associated with tibia fracture. 9 patients had distal fibula, 14 had posterior malleolus, 21 had lateral malleolus and 4 had medially malleolus fractures.

Results: The results revealed that there is no statistically significant correlation between the mechanism of injury and ankle fracture, and that the level of tibia fracture, in a similar way, is not related with ankle fractures. This study showedthat ankle fractures occur more frequently with spiral shaft fractures of tibia.

Conclusions: Ankle fractures can be simultaneously seen with tibia fractures regardless of the etiology and location of the tibia fracture. Also, as this study reveals, this association was more likely in the presence of a spiral shaped tibia fracture.
