THE RELATIONSHIP BETWEEN INSULIN RESISTANCE AND VITAMIN D DEFICIENCY WITH BLOOD PARAMETERS

Öz

Aim: To investigate HOMA-IR and Vitamin D levels in patients complaining of weight gain and to idendify their relationship with blood parameters taken for examination.
Materials and Methods: There were a sum of 99 participants in the research (nmale = 25, nfemale = 74). Fasting glucose and insulin, cholesterol, complete blood count, aspartate/alanine transaminase, thyroid stimulating hormone and 25-OH D levels were studied. Two different groups were created in the study. By calculating HOMA IR; those > 2.5 and above were placed in the insulin resistance group, while those < 2.5 were included in the non-insulin resistance group. In the Vit D classification, those with 25 OH D < 20 were categorized as deficient, and those with ≥ 20 OH D were categorized as sufficient. The correlation of the groups with blood parameters regarding these classifications was identified.
Results: BMI, ALT TRG, Ferritin, FBG, INSULIN, HBG and HCT values were significantly higher in Insulin Resistance group, while HDL values were lower (p < 0.05). Positive correlation was noticed between HOMA IR and MPV (p=0.289; p=0.04). BMI was significantly higher and a significant negative correlation was observed with FBG and TSH in VitD deficiency group (p < 0.05).
Conclusion:HOMA IR was associated with increased BMI and fasting glucose in both groups with >2.5 and 25 OH D <20. However, while HOMA IR was positively related to MPV, 25 OH D was negatively associated with TSH.

İNSÜLİN DİRENCİ VE VİTAMİN D EKSİKLİĞİNİN KAN PARAMETRELERİYLE İLİŞKİSİ

Öz

Amaç: Bu çalışmadaki amacımız kilo artışı şikayetiyle başvuran hastalarda HOMA-IR ve Vit D düzeylerini incelemek ve tetkik amaçlı alınan kan parametreleriyle ilişkisini araştırmaktır.

Materyal & Metot: Çalışmaya 99 katılımcı dahil edildi(erkek=25, kadın =74).Tetkik olarak açlık glukoz ve insülin, kolesteroller, tam kan sayımı, aspartat/alanin transaminaz, tiroid stimülan hormon, 25-OH D düzeyleri çalışıldı. Çalışmada 2 farklı grup oluşturuldu. HOMA IR hesaplanarak ; > 2,5 ve üzeri insülin direnci grubuna alınırken, < 2,5 olanlar ise insülin direnci olmayan gruba alındı. Vit D sınıflaması ise 25 OH D < 20 olanlar eksiklik, > 25 OH D olanlar yeterli olarak kategorize edildi. Grupların sınıflamaya göre kan parametreleriyle korelasyonu incelendi.

Bulgular: HOMA-IR’ye göre gruplar incelendiğinde İnsülin Dirençli grupta VKI,ALT TRG, Ferritin, AKG, İnsülin, HBG ve HCT değerleri daha yüksek bulunurken, HDL değerleri daha düşük bulundu (p< 0.05). HOMA IR ile MPV arasında anlamlı pozitif korelasyon saptandı(p< 0.05). 25 VitD yetersiz grupta VKI daha yüksek bulundu ve AKG ve TSH ile anlamlı negatif korelasyon izlendi (p< 0.05).

Sonuçlar: İnsülin dirençli ve Vitamin D yetersizliği olan grupların her ikisi de artmış VKI ve açlık glukozuyla ilişkili bulundu. Bununla birlikte HOMA IR’ın MPV ile pozitif yönde korelasyonu saptanırken, 25 OH D’nin TSH ile negatif ilişkisi bulundu.
