Carpal tunnel syndrome secondary to space occupying lesion is a relatively rare condition. Median nerve compression secondary to carpal osteophyte is extremely rare. We report a case of carpal tunnel syndrome due to osteophytic bony projection in the carpal tunnel in a 54-year-old man with antecedent traumatic wrist injury before 20 years. To our knowledge, this is the second reported case. Complete excision of osteophyte resulted in complete relief of sensory symptoms.
